In patients who need 3rd or 4th repeat Cesarean sections, one should be careful in terms of upper rectus sheath dissection to create para-rectus sheath space for delivery of the baby. The dissected sheath will continue to get weaker and weaker after each episode.
Finally there is always a chance that patient may be left with button hole defect, which can be easily missed and later presenting as incisional hernia. The hernia bulge will be in the midline away from the pfannensteil scar.
This blog is of my personal experiences in the labour ward and also in the clinical setting. This mainly represents the challenges I had and the solutions I came up with. I believe similar work challneges are faced by my colleuagues too.
